Service Mission
To provide professional and transitional service support to persons in mental recovery who are prospective or current service users of halfway house (HWH) through the adoption of the recovery model with medical-social collaboration, with a view to helping service users reintegrate into the community.
Service Objective
To devise and implement comprehensive recovery plan customized to needs of service users by adopting a strength-based approach along the positive and autonomous recovery process, in order to develop individual capacity, build support network and reintegrate into HWH/ community living to accomplish personal goals.
Target Groups
Persons in mental recovery living in our service area, including:
- Potential service users of HWH who have been waitlisted for HWH service at CRSRehab-ExMI;
- HWH services users who have already completed trial stay but left HWH service pre-maturely for various reasons; and
- HWH service users who have been residing for at least 36 months and have difficulties in discharge.
Service Area
Central and Western, Southern, Eastern, Wan Chai, Kwun Tong, Kwai Tsing [Tsing Yi and Kwai Chung (selective parts)]#
#Kwai Tsing includes:
- Tsing Yi Island;
- Kwai Fong Estate, Kwai Tsui Estate and Tai Lin Pai Road Factory area
- Kwai Chung Plaza, Sun Kwai Fong Gardens, Kwai Fong Court, Ha Kwai Chung Village, Hibiscus Park and the private housing area near Shing Fong Street, Wing Fong Road, Kwai Hong Court and Ko Fong Street, Kwai Hong Court
- Lai Yiu Estate, Tsui Yiu Court, Kau Wah Keng San Tsuen, Kau Wah Keng, Lai Yan Court, Lai Wan Garden, Wah Lai Estate, Happy and the other private buildings around Chung Shan Terrace
- Wah Yuen Chuen, Regency Park, Wonderland Villas and Greenknoll Court (excluding Nob Hill)
Scope of Service
To design individualized recovery plan by utilizing service users’ personal resources and strengths through the following services:
- Outreaching service
- Counseling service
- Individual recovery training plan
- Occupational therapy assessment and training
- Training for community adjustment/living skill
- Family aide service
- Peer support service
- Therapeutic and supportive group
- Carer support
- Bridging/Support service of HWH and ICCMW
- Referral service
Feature of Service
- Medical-social-family collaboration: maintain communication with referrer, carers, medical officers, community psychiatric nurses, and medical social workers to understand needs of service users to provide holistic and systematic assessment and follow-up
- Multi-disciplinary team follow-up: social worker, occupational therapist and psychiatric nurse and peer support worker together provide tailor-made service for service users
- Strength-based approach: service users are encouraged to be involved in designing individual recovery plans customized to their needs and desires
- Flexible intervention: outreaching service allows the team to approach service users at places they are familiar with and in ways which are convenient for them; and bridge service users to related services
- Holistic care: the project promotes holistic development in physical, mental, social and spiritual health, and emphasizes on the support offered during transition
Application Procedure
- Self-approach: Target service users and/or their family members/carers may visit the service centre (with prior telephone appointment) to apply for service; or
- Referral: Contact with referrer (medical practitioners and social workers of HWH/other community institutes - e.g. ICCMW) and arrangement of intake will be provided upon written referral.
Exit Procedure
Service users and their family members/carers are entitled to apply for termination of service. Our staff will contact the relevant parties to understand the reasons for termination and arrange for the exit of service, including the timeline of service termination and making referrals to other relevant services as necessary.
Our services will be terminated if service users:
- requested for termination of service; or
- are in serious violation of the rules of the service unit, e.g. behavior causing nuisance and jeopardizing the well-being of other service users or our staff. Repeated failure to comply with rules of the service unit may result in the service users’ disqualification.
Service Fee
Services in the scope of the Project under the Funding and Service Agreements are free of charge
