Service

Co-initiated and Co-funded by Ho Cheung Shuk Yuen Charitable Foundation, Chow Tai Fook Charity Foundation and Shih Wing Ching Foundation, and invited Sau Po Centre on Ageing of The University of Hong Kong as a knowledge partner, Baptist Oi Kwan Social Service (BOKSS) Integrated Elderly Services has launched a three-year project named as “Dementia Hand in Hand Project” in two public housing estates, Cho Yiu Chuen and Lai Yiu Estate. It affirms that every caregiver should be respected, valued and supported. We not only advocate the concept of “Caregiver Rest Day", but also provide comprehensive services to the elders and their caregivers as a whole through early identification, dementia care planning, respite service and digital training program. We are also forming community alliances to raise awareness on the need of carers in the community and improve their needed facilities. After this three-year multi-step project, it is hoped to provide timely and planned support to the caregivers, in the meantime, to build up a dementia- and carer-friendly community in the Kwai Chung district, Hong Kong.

Aims

1) To enhance the quality of life of persons with dementia (PwD) and their caregivers;

2) To provide timely and appropriate support to the caregivers; and

3) To promote dementia and carer friendly community in the district


Target Group

In Cho Yiu Chuen and Lai Yiu Estate, those supporting to or with dementia at early stage or mild cognitive impairment (MCI) and may be in need of respite and supportive care

About the Project:

 

1) Individual level:

 
To enhance the quality of life and self-reliance of people with dementia throughout different stages of dementia through early identification, dementia care planning, digital training program and respite service.

2) Family Level: To enhance the quality of life of carers throughout the journey of caregiving by introducing carers’ need assessment, carers’ wellness plan, peer support program, use of technology and respite service.

3) Community Level: To build up dementia- and carer-friendly community, which understands, respects and supports the need of people with dementia and their carers.

Research Team: The Evaluation Study co-worked with Sau Po Centre on Ageing of The University of Hong Kong, aiming to:

1) evaluate the planned respite service (feasibility, acceptance, and impact on caregivers);

2) propose intervention strategies to reduce perception discrepancies between people with dementia and stakeholders;

3) evaluate the outcome of the intervention.
