Service Objective
To provide multi-disciplinary transitional support service to Persons in Mental Recovery (PMRs), whom are prospective service users of halfway house, by adopting the recovery model and fostering medical-social collaboration, for the prospect of helping service users in adapting to new accommodation and pursuit stable living.
Target Groups
Potential service users of HWH who have been waitlisted for HWH service at under CRSRehab-PMR of the HWH service
Service Area
Central and Western District, Southern District, Islands District, Eastern District, Wan Chai District, Kwun Tong District, Wong Tai Sin District, and Sai Kung District
(According to the residential address of service users)
Scope of Service
By designing individualized recovery plan, to utilize service users’ personal resources and strengths to enhance the capacity to adapt to the challenges and stress in the transitional period.
Outreaching service covering the following aspects is provided:
- Bridging/Support service for HWH or community living
- Training for HWH/community adjustment and living skill
- Emotional support and counseling service
- Individual recovery training plan
- Therapeutic and supportive group
- Carer support
- Peer support service
- Referral for other community support or rehabilitation service
Feature of Service
- Medical-social-family collaboration: maintain communication with referrer, carers, medical officers, community psychiatric nurses, and medical social workers to understand needs of service users to provide holistic and systematic assessment and follow-up
- Multi-disciplinary team follow-up: service delivered by multi-disciplinary team, including social worker, psychiatric nurse, etc., with peer support workers, to provide tailor-made service for service users
- Strength-based approach: service users are encouraged to be involved in designing individual recovery plans customized to their needs and desires
- Flexible intervention: outreaching service allows the team to approach service users at places they are familiar with and in ways which are convenient for them; and bridge service users to related services
- Holistic care: promotes holistic development in physical, mental, social and spiritual health, and emphasizes on the support offered during transition
Application Procedure

Self-approach: Target service users and/or their family members/carers may visit the service centre (with prior telephone appointment) to apply for service; or
Referral: Contact with referrer (medical practitioners, social workers, or allied health profession) and arrangement of intake will be provided upon written referral.
Exit Procedure
Service users are entitled to apply for termination of service by written format, by phone, or by appointment. Our staff will contact the service user, carers, and/or relevant parties to understand the reasons for termination and arrange for the exit of service, including the timeline of service termination and making referrals to other relevant services as necessary.
Remarks: When the service objectives are achieved (e.g. living in Halfway House for 6 months, or settled in community or having bridged to appropriate community service, the service will be terminated
Service Fee
Services in the scope of the Project under the Funding and Service Agreements are free of charge.
Some groups/activities might be subjected to additional charges. For details, please refer to announcement or approach staff for details.
